Health Checkup
Part of the medical examination form can only be filled out by a doctor
after having given you a medical checkup. The checkup is just a basic
physical (e.g. height, weight, sight test etc), taking a maximum of an hour.
Be sure to take the examination form with you when you go to the hospital.
The doctor will fill-out the checkup portion and endorse it.
Note: Such health checkups are only accepted if done in designated
hospitals. (name list in Beijing).
Expatriates who do not wish to go to the Vehicle Office in person for test
time appointment would authorise us for the purpose with the following
documents,
• Passport with valid visa
• International Driving License and the translated copy
• 4 one-inch coloured photos against pale-blue or white background
• Residence Permit
• Application form ( must be filled out in Chinese, the backside is the
instructions, download the form here )
• Medical examination form ( take the form with you when going to the
hospital, download the form here )
